Diabetes is epidemic in the United States having grown from 5.6 million in 1980 to 16.8 million by 2006. Another 26% of adult Americans have a prediabetes and thus are at increased risk of developing diabetes, along with comorbid cardiovascular disease. This presentation will address the development of the SD model, how it has been used to date, and future steps.
